Missions principales du poste

Sous la responsabilité de la Directrice de Pôle et en coordination avec la Cheffe de service :

Vous évaluez avec la personne son état de santé global et faites émerger les besoins
Vous participez activement aux différentes réunions d’équipe
Vous participez à la mise à jour des dossiers médico administratifs des personnes accueillies
Vous êtes en veille s’agissant de l’état clinique et psychique des personnes accueillies
Vous contribuez à maintenir une autonomie des personnes accompagnées en développant leurs
connaissances et leurs compétences
Vous vous appuyez sur les compétences d’une équipe pluridisciplinaire pour garantir l’élaboration, la
formalisation, la mise en œuvre et l’évaluation des projets personnalisés en sollicitant la contribution
de chacun des professionnels des services
Vous intervenez en transversalité sur différents services sociaux et médico-sociaux.

Aide soignant.e

Expérience auprès des publics en situation de grande précarité souhaitée Bon contact, dynamisme et facilités relationnelles

Capacités d’écoute, d’organisation, de gestion du temps, d’autonomie

Capacités à analyser les situations et les interlocuteurs, à traiter et transmettre l’information

Qualités rédactionnelles - Maitrise du pack office

Travail en week-end et en soirées possible, maraudes

Permis de conduire catégorie B exigé
